Empiric MRSA coverage may be included in patients with risk factors for MRSA infection. Risks inculde:
- MRSA nasal colonization
- recent hospitalization
- incarceration
- necrotizing pneumonia or empyema (cover for anaerobes for pulmonary abscess and empyema) (pulmonary cavitary lesions do not need anaerobic coverage)
- recent antibiotic use
- previous MRSA infection
- IVDU
- Do not need MRSA coverage for urosepsis or intra abdominal infection
- Need MRSA coverage for line infection, endocarditis, soft tissue, pneumonia1
