Observational data suggest that intubation in the ramped position over the supine, sniffing position may decrease hypoxemia in patients who are morbidly obese. However, first-pass success rates were lower, and the lowest SpO2 was similar in a randomized trial of ramped vs supine positioning for endotracheal intubation, with higher incidence of Cormack-Lehane grades 3 and 4 of the glottic view in the ramped position.2
