stress dose steroids dosing and tapering

  • start stress dose steroids for sepsis patients on leveophed and vasopressin. Start with IV hydrocortisone at 50 mg q6h
  • taper stress dose steroids once patient is hemodynamically stable off pressors
  • taper by either changing to 25 mg q6h or 50 mg q12h
  • steroid equivalents
