use beta hydroxybutyrate, ketones, and not pH to diagnose DKA


  • patients can have DKA and have normal pH and bicarb
    • if they have vomiting, they can also have concomitant metabolic alkalosis
  • using urine ketones and elevation in beta hydroxybutyrate is a better way to assess DKA.1
