amiodarone dosing for atrial fibrillation

Loading

  • start with IV bolus 150. May need 2-3 boluses
  • Then 1 mg for 6 hours, followed by 0.5 mg per min for 18 hours
  • load with 10g of amiodarone
  • Can stop infusion after 24 hours
  • amiodarone PO: 7-8 grams
  • 400 mg BID until 10 g dosing, and then 200 mg daily
  • 400 BID x 7-10 days = 10 gram
  • then 200 daily until follow up outpatient
