Cryptogenic organizing pneumonia is characterized histologically by the presence of just organizing pneumonia. When organizing pneumonia is idiopathic, the patient may be diagnosed as having cryptogenic organizing pneumonia. Organizing pneumonia is the presence of loose polypoid plugs of granulation tissue in terminal and respiratory bronchioles, alveolar ducts, and sacs. These lesions are called Masson bodies (Figure 6). This is demonstrated in the second option and is not consistent with the clinical story or images provided (choice B is incorrect).1
