DAH bronchoscopy shows hemosiderin laden macophages


Bronchoscopy with bronchoalveolar lavage (BAL) is a key step in determining the cause of DAH, as it helps exclude infection and other processes such as acute eosinophilic pneumonia. BAL specimens should be sent for cell and differential counts; routine bacterial, mycobacterial, fungal, and viral cultures; and Pneumocystis stains. The diagnostic BAL finding of DAH is persistent or increasing blood on three sequential lavage aliquots from one affected area (Figure 4) and more than 20% hemosiderin-laden macrophages in the BAL fluid (Figure 5).1
