Randomized studies have demonstrated that extubation immediately to NIV (termed “prophylactic NIV”) in patients at high risk for reintubation reduces the incidence of failed extubation and need for reintubation, especially in patients with known hypercapnia. Additional randomized trials have also demonstrated that NIV can rescue patients with hypercapnia who experience postextubation respiratory failure, even when prophylactic NIV was not used.123245
