nitroprusside

  • converted to cyanide via kidney and then thiocyanate in liver
  • thus either liver/kidney dysfunction can lead to toxicity
  • cyanide toxicity: elevated lactate and AMS
  • treatment: IV thiosulfate

pregnancy

  • over 32: induce vaginal delivery

pheochromocytoma

  • goal systolic less than 140
  • abrupt increase in BP
  • can have more profound end organ damage and mental status changes
