systemic heparin is used as anticoagulation in CRRT


  • Most centers do not start with anticoagulation in CRRT.
  • anticoagulation otions include regional citrate and heparin
  • citrate is not approved for CRRT in US
  • citrate causes less bleeding but requires change in dialysate
    • decrease buffers (lactate and bicarb) since citrate is a buffer
    • increase mag concentration since citrate can bind mag
  • unfractionated heparin is usually used.1
