Treat both acute and chronic eosinophilic pneumonia with steroids. Use IV to start and then switch to oral. You should see response in CXR and eosinophilia 2 days, if not look for other diagnosis. Treatment duration is 3 months followed by 3 months taper.1
Treatment is with corticosteroids at relatively high doses (methylprednisolone 240-1,000 mg each day in divided doses has been suggested) for respiratory failure, and oral prednisone for those not acutely ill, or as acute illness resolves, with a rapid taper over a 2to 4-week period. Patients usually have a rapid and dramatic response, usually within 1 to 3 days. In contrast to CEP, relapse in AEP is rare.2
