Fundamentals of High Resolution Lung CT
Hypersensitivity pneumonitis
- HP hypersensitivity pneumonitis
- p186
- HP is result of immune reaction to organic inhalants
- acute HP is rare: moldy hay in farmer's lung is an example
- subacute HP most common in weeks to months
- histologically represented by inflammation
- chronic HP causes fibrosis
- bronchiolocentric and poorly formed granulomas a compared to well formed in sarcoidosis
- p187
- subacute HP image finding includes GGO, centrilobular nodules, mosaicism, headcheese sign (combination of GO and mosaicism), mid lower lung distribution, spares costophrenic angles
- centrilobular nodule reflects peribronchovascular inflammation, poorly marginated granuloma, cellular infiltration
- p188
- centrilobular nodules usually associated with low attenuation areas of mosaic perfusion (headcheese sign)
- p189
- distribution usually in mid lung and more central as compared to peripheral and lower lung in other interstitial pneumonias
