Pathogenesis

• Common bugs: strep, H. flu, mycoplasma, chlamydia, legionella
• alcoholism: klebsiella
• Use Shorr score to calculate MRSA pneumonia risks. Could also use MRSA nasal swab

Diagnosis

• use of urine legionella antigen and pneumococcal antigen useful?
• sputum culture usefulness?
• colonization vs new infection?

Treatment

• CAPE COD trial shows steroid reduces mortality in severe pneumonia patients
• Add vancomycin for patients at risk for MRSA pneumonia, including those with influenza virus
• Inhaled Amikacin to Prevent Ventilator-Associated Pneumonia
• CAP antibiotics
