Overview
- shock is rarely from one cause. Even when it starts as either cardiogenic or septic, it will usually involve other types of shock in later stages
- septic shock can cause septic cardiomyopathy
- cardiogenic shock can cause bacterial translocation in GI track
Diagnosis
- both tachycardia and bradycardia can point to shock
- elevated shock index can suggest shock
- severe bradycardia can result in decreased cardiac output
- oliguria: low urine output is less than 0.5 cc per kg
- mottling of skin suggests active vasoconstriction and could have benefit from inotrope not more vasoconstrictors
Types of Shock
Major types of shock are as follows: cardiogenic, distributive, obstructive, hypovolemic
However, it may be easier to remember from head to toe:
- neurogenic
- trauma
- spinal anesthesia
- cardiac
- LV or RV failure
- MI
- tamponade
- LVOT obstruction
- PE
- tachycardia or bradycardia
- Post arrest or MI SIRS
