Diagnosis

• diagnosis of asthma includes typical symptoms and variable airflow limitation
• repeat PFT when symptomatic or withhold bronchodilators to diagnose patients suspicious for asthma with normal lung function
• diagnosis of asthma in patient already on controller therapy

• mannitol inhalation test for asthma diagnosis
• aspirin exacerbated respiratory disease

Symptoms

• asthma in pregnancy are divided into 3rds
  • 1/3 same, 1/3 improved, 1/3 worse
• use forced expiration to elicit wheezing in asthma patients

Assessing Asthma Control

• assess asthma symptoms over 1 month period and PFT every 3 to 6 months
• mild asthma, severe asthma, difficult to treat asthma

Treatment

• inhaled corticosteroid equivalents
• step up or down treatment in patients with increased or decreased symptoms
• asthma step up therapies
• SABA is not recommended for treatment of mild asthma
• use ICS formoterol as initial asthma treatment
• add LAMA to asthma treatment improves lung function but not symptoms
• Moores triad of asthma management
• acute asthma exacerbation
• long prednisone taper for asthma exacerbation
• continue normal asthma management except ICS LABA in pregnant patients
• treating asthma with bronchodilator acutely does not treat inflammation
• use bronchial thermoplasty to treat severe asthma
• dupilumab dupixent is used to treat oral corticosteroid dependent asthma
• dupilumab dupixent can have transient elevation in blood eosinophils
• start allergen immunotherapy for controlled asthma patients

Asthma in ICU

• decrease flow rate in status asthmaticus over increasing PEEP
